On Sunday, April 3rd, 1892 at 11:56 AM, an alarm was sounded on Box 963, Broad Street & Lehigh Avenue (Station House Box), for a fire in a two-story brick dwelling occupied by J. Smalley, located at 1505 Oakdale Street. While responding with his company, Hoseman William Barnes of Engine Company 12, who was detailed to Engine Company 30, was killed when his neck was broken after being thrown from the hose cart on which he was riding.

The cause of the fire was listed as “Defective flue”.
