While operating at a dwelling fire at 2240 Orianna Street, Firefighter Casiano fell through a hole in the floor. After being helped from the hole he continued with his duties.

After operations were complete, Engine 2 returned to the station. Firefighter Casiano complained of not feeling well. He said, “It feels like I swallowed something.” Almost immediately, Engine 2 received another run.

Firefighter Casiano remained behind. When the company returned about a half-hour later, they found him unresponsive on the floor. CPR was started immediately. He was transported to Episcopal Hospital where efforts to revive him failed.

The cause of death was determined to be internal bleeding resulting from his fall through the floor.

Firefighter Casiano was cited for bravery in 1994.
